• Requesting Party – Along with the service company info should be the client’s personal and contact details.
• Service Details – This will relay where and when the service should be carried out, who will be responsible, who received the request, etc.
• Order Details – For administrative purposes, the form should list the order date, a reference number, and the level of urgency.
• Labor – A description of the labor required along should be listed as well as the number of hours, the cost of the serviceperson per hour, and the total cost of labor.
• Materials – The document should also list the materials required to carry out the service and their associated cost.
• Cost  – The total cost of labor and materials can be added including tax. Cost can be reflected as an estimate or to keep track of actual costs after the fact.
