What to Fill In

  • Daycare Info – This includes the name, address, contact info, and EIN.
  • Client Info – The name of at least one parent and the name and age of the child.
  • Services – The type of service (child care) and the term (what months/dates).
  • Costs – The breakdown of the cost of services, additional fees, and total with tax.
  • Amount Paid – Clear statement that the payment has been received.
  • Provider signature – Often expected if the parent has an FSA account.

Why Child Care Receipts Are Necessary

Receipts are needed to prove parents actually paid for services. Because parents can reduce their annual tax burden through the Child and Dependent Care Services Tax Credit or a Dependent Care FSA, centers and in-home caregivers need to be ready to provide documentation that shows:

  1. Expenses were paid.
  2. They were for actual child-care services.
  3. The care was necessary for work-related purposes.
