How to Fill Out a Donation Receipt

• Organization – The name, member food bank location, address, and federal tax ID number of the charity must be provided.
• Details – Enter the receipt number, donation date, and the name of the Feeding America representative that received the contribution.
• Donor – The donor’s name, address, and phone number should be included in the donor section.
• Contributions – List all the goods (plus quantity and weight) or monetary contributions that were made by the donor. Only enter value for cash donations.
• Acknowledgment – The individual who received the donation can sign, date, and print their name at the bottom.
