It allows patients to see what procedures are planned and understand how the price is calculated. Dental estimates are not guaranteed, and the final cost may change depending on the actual work performed and the amount covered by insurance, if any.

Essential Estimate Inclusions

Provider Info – Facility’s name, address, national provider identifier, and taxpayer ID.
Patient Details – Patient’s name, date of birth, and contact information.
Services – Description of each reasonably expected dental service or item.
Codes – Diagnosis codes (if determined) and applicable service or procedure codes.
Fees – Estimated charges for the planned dental work.
Coverage – Amount expected to be paid by insurance (if applicable).
Out-of-Pocket Costs – Total amount the patient is likely to pay out of pocket.

Important Notice for Uninsured and Self-Pay Patients

Patients who are uninsured or choose not to use insurance must receive a Good Faith Estimate (GFE) outlining their expected treatment costs.^[1] A GFE differs from a standard dental quote because it must include specific legal disclaimers and be provided within federally required timeframes after scheduling treatment or upon the patient’s request.

The Centers for Medicare & Medicaid Services has provided a sample Good Faith Estimate form that includes the required disclosures and detailed instructions for its use.
